Not sure where to go...

I am going to fly to Ireland where my family lives. I have an Emotional support dog and my trip is about 5-6 weeks long.
How do I get a passport for my dog? How do I know which airlines will let me bring him with me? I also am going to Amsterdam for a week in that time too! So if someone could help me out I would be most obliged!
